Cancellation and Rescheduling Policies. Please read before submitting your registration.

Please make sure that you can attend the livestream events as they an not on-demand style course. Study materials for the courses will be provided during the week after completion of the module. Generally, they are provided on the Monday following the weekend.

If you register for any of the Modules 1-3 and find you cannot attend the livestream please contact Mary directly at [email protected] as soon as you know. You may apply your registration fee to a future course with Fabian Brotos hosted by mindbodylogic or you may find someone to take your place. If a future online or in person course is not an option due to health reasons, we will do our best to accommodate you while respecting Dr Brotos' time and energy on a case by case basis. If you contact us at least 2 weeks prior to the scheduled online event you are eligible for a partial refund your registration minus a $40 administration fee. No refunds or consideration will be given after the start of the course for which you have registered.

Registration for the Module 4 Retreat is refundable minus a $100 administration fee until March 15, 2022. After such time your course registration may be transferred to someone of your choosing that will be qualified to attend Module 4 by having participated in at least one of the first 3 Dental Decoding Modules prior to the start of Module 4. Qualification can be confirmed from any of Fabian's international Modules 1-3 provided the attendee can converse and understand English. Cancellation after this date is non-refundable but may be transferred to future Dental Decoding modules hosted by mindbodylogic.

All requests for refunds are to be done in writing by email to Mary Kennedy [email protected]. Allowed refunds will be processed electronically within 14 business days.

Note: There are no refunds outside the refund policy for these events. If a speaker listed on the schedule is
unable to attend due to an Act of God, we will do everything in our power to find a replacement for that
speaker.

Travel Insurance is highly recommended. We reserve the right to cancel an event due to low enrollment,
extreme weather, or other circumstances which would make the event non-viable. Refundable and changeable flights are recommended.

If the event is canceled for any reason, you will receive a refund minus a 7% administrative fee within 30
business days. Should circumstances arise that result in the postponement of an event, participants will
have the option to either receive a refund or transfer registration to the same event at the new, future date
if available. The organizer bears no responsibility for travel arrangements or any other actual or perceived
loss due to cancellation of the retreat. Travel Insurance is highly recommended.

COVID-19 CANCELLATION POLICY 10% Administration fee for In person event, $25 for LiveStream.

Participants seeking refunds due to COVID-19 HIGHRISK OR CONTRACTING consent to the disclosure of
private, confidential medical information/records to process a refund claim due to contracting COVID 19, and further
agree that the documents/information submitted complies with HIPAA’s rules and regulations.
Documents required for refund claim a letter, declaration or affidavit from a licensed medical provider.

EXTRAORDINARY CONDITIONS CANCELLATION POLICY FULL REFUNDS
Sudden injury; a pregnancy emergency; an emergency surgery, or similar unplanned medical procedure within
the seven (7) days prior to the event, death in immediate family.

Consent to the disclosure of private, confidential medical information/records to process of a refund claim due
to Sudden injury; a pregnancy emergency; an emergency surgery, or similar unplanned medical procedure within the
seven (7) days prior to the event, and further agree that the documents/information submitted complies with HIPAA’s
rules and regulations.

Documents required for claim: In the case of an emergency medical procedure, a letter, declaration or affidavit from
a licensed medical provider, or a treatment record from the procedure itself. In the case of death, a copy of the death
certificate or other official documentation confirming the date and time of death.
